JT Connected Care

JT Connected Care offers a 24-hour support service for people living in Jersey, enabling ageing, disabled or vulnerable islanders to remain living independently in their own homes using connected technology.

We will be launching with our first package to cover falls and reassurance, with additional service levels to follow.

What is JT Connected Care?

JT Connected Care is a cloud-based, real-time connected health and care monitoring system. The system utilises digital connectivity (our local fibre and mobile data networks) and Smart Hubs to empower freedom and flexible independent living for vulnerable users by delivering efficient, proactive, preventative, and coordinated community care.

Falls & Reassurance

Independence is something that we all cherish. However, there comes a time when you may need a bit of support to live in your own home.

Our first comprehensive falls and reassurance package gives you the benefits of a 24/7 monitoring service, using advanced fall detection technology and supporting you to live independently in your own home and giving your family peace of mind that help is at hand no matter what the emergency.

The wearable falls sensor, when activated by a fall or pressing the button will alert our friendly experienced monitoring centre staff to triage your call efficiently and effectively to contact, in the first instance, your next of kin, carer, friend or neighbour.  If appropriate, the service can also contact your GP or even the emergency services if you are injured or hurt.

All our service users will have a key safe fitted as standard to allow your responder’s non-instructive access to your property.

What you get with your subscription

Smart Hub and necessary sensors will be demonstrated, installed and explained.

We will provide a pack explaining what the service is about and how it works.

On-going monitoring will provide reassurance and peace of mind for you and your family.

You will have access to a 24-hour monitoring centre that’s tailored for your response requirements.

We will call you annually to update your data and verify your requirements.

On-going support from the JT team for your equipment, any faults will be attended to within 24 hours.
