    Notice of Bereavement

I would like to close the account of someone who has passed away.

We're sorry to hear that someone has died. Thank you for taking the time to let us know. We will try to make this as easy as we can for you.

You'll also need to tell us what you’d like us to do with the account, for example transfer the services into another name or close the account.

How to tell us

  • You can tell us online or in-store.
  • You must be the Executor of the will.
  • You'll need the account holder's name, date of birth, address and account number.
  • You’ll need to show us a copy of their death certificate and your ID.

Online

Use the online form to tell us about an account holder's death.

In-store

Our advisors in-store will help you to complete the online form. Our retail opening hours are 9am to 5.30pm Monday to Saturday.

What happens to the account

You'll also need to tell us what you want us to do with the account, for example transfer the services into another name or close the account.

APPENDIX A – TERMS AND CONDITIONS

The signatory detailed in Section 2 above

  • Mobile Device Payment Plans are not able to be taken over.
  • Should a Service be taken over which has a Mobile Device Payment Plan attached to it, the remaining Mobile Device payments due for the remainder of the Payment Term will become payable.
  • Take over of contract and financial liability for subsequent bills can only take affect immediately at point of processing in store.
  • JT One packages (all 3 Services) can be taken over at any point, however, the package can only be split and Services taken over if they are out of contract term. In this instance, the other Services would automatically be changed onto current standard plans.
  • Take over completion is subject to incoming Customer passing JT’s credit check and being accepted.

Incoming Customer

  • Acceptance of terms and conditions and financial liability of all remaining Service contracts relating to Services take over as agreed by the signatory detailed in Section 2 above.
  • Take over of contract and financial liability for subsequent bills can only take effect immediately at point of processing.
  • Take over completion is subject to incoming Customer passing JT’s credit check and being accepted.
